Personal data request for Consumers & Parents/Guardians 


Please use this personal data request form if you are: 

  • An individual Consumer who has or had a personal account with T-Mobile, Metro, T-Mobile Prepaid, Connect by T-Mobile, or Assurance Wireless.
  • A Parent or Guardian of a child who has a designated Kids’ Line with any of our brands
  • A Parent or Guardian of a child who has an account with any of our brands.
  • An Authorized Agent of an adult who has an account with any of our brands.
  • A Consumer who has never been a customer of one of our brands.
